In need of help...clam deaths.

3 clam deaths in 3 weeks, 2 last night. One the clam was completely cleaned out last night. I can't figure this out. Predation, or a clam death from something else and something cleaned out the shell. The other death I witnessed last night I found a giant flatworm at the scene which I removed. Does anyone have any insight into this?
I really need some help.

Tridacna and Hipopus clams can die for many reasons from poor lighting and bad water conditions to parasites and predators. They should also be fed a good formula of phytos to keep their health up - they do have parts that enable them to filter feed. Smaller clams need more feeding and usually are more sensitive and are harder to keep alive.
 
Two died in one night...All of my water params are perfect zero nitrate, zero ammonia, zero phosphate. Alk 11dkh Ca 390. They are fed regularly. I found a flatworm and removed it upon the first clam death. This is listed in the thread I attached. I woke up this morning and another clam had been completely cleaned out. I can't afford to wait until the lights go off tonight for this to happen again. Please someone who is a clam expert, please advise based on what I have posted. What are the chances that there are more of these flatworms? How do I remove them short of exit?
 
Have you posted a pic on Dr. Rons forum? Are you sure they are not any crabs in your tank. I'd check the tank out in the dark with a flashlight. Thats very strange.
