Is this Ich?

It could be, but more info is needed. What is the history of this fish, as well as the tank and its occupants? Was it quarantined? If so, describe method and length of QT period. Any unusual behavior, like scratching, twitching, head-shaking or labored breathing?
 
300G DD with 15 fish roughly. Tangs, gobys, angel etc.
Tank had ICH 2 1/2 months ago due to a new addition, a lieutenant tang. I know I know I should have QT'd the Tang.
Removed all fish, treated with copped safe for 3 weeks in a 36G QT Tank. Then made water changes to the QT tank for the next 3 weeks. At the last 2 weeks I used the main tank water for changes for acclimation.

In regard to the main 300 tank I added kordon rapid cure and treated for 3 weeks, raised temp to 81. The reef was fine with the tratement, no loss. After water change I ran my emperor HO 80 watt smart uv sterilizer 24x7 for two weeks.

Well unfortunately...
1) using the tank water wasn't a good idea because it possibly contained crypt still, thus potentially reinfecting the fish.
2) the Kordon Krap doesn't do anything to cure the tank. You have to run the tank fish less for 72 days.
3) raising the temp doesn't do much against marine ich.

Looks like it still may be in your tank.
 
In a tank with ich, lowering temperature is preferred; does not affect ich but raises dissolved oxygen.

- pull the water out of the water column as far from any hard surface as possible. Do this while the lights are on and ideally have been on for at least 8h.
- store the water for 24h
- transfer the water into another container and store it there for another 24h.

After this the likelihood of infective ich stages in the water is as close to "0" as you will ever get.

It may of course be easier just to use new water.
