My Purple tang did the same thing recently. I started giving Nori once a day instead of twice a week and he is back to normal.

Yes, it is a grazing issue. However you have a 150 gallon reef which is a much better situation than the OP so is more easily fixed.

My yellow tang is only about 2 1/2 inches max , so he's fairly small I think in a 90 gallon 4 ft tank that's plenty for him at this stage , it's not like I'm keeping him in a nano cube.
He's getting norri once a day now aswell
