Jumping Dwarf Angel ?

Last night my Coral Beauty jumped out of my tank for her last time:sad1:.This is the second time she jumped last time I had glass on the top and she managed to jump through the small opening and flop around on the glass for a minute but my wife got her back in the tank,this time my son found her on the floor.I had taken the glass off because I heard fish might jump if oxygen level was low and have read covering the top might cut down oxygen.My questions are has anyone had a jumping Angel?Is it typical(i can't find any posts on it)?Should I get a screen top before I replace her? I should add I've had her 10 months or so and It only happened twice and sometimes she "plays"near the top(i've had her splash my when she wanted fed).I understand things happen but want to take precautions should I replace her. thanks

Precaution would be a mesh top that is kept on.

+1 I have this on my tank and it works well. make sure you get the 1/4" from BRS. I think any fish has a tendency to jump really, just a matter of when.
