Kole tang in trouble

My Kole tang is pretty much staying in one semi secluded are of the tank. I noticed yesterday he didn't come over to feed on the Nori when I put it in. He has a spot on both the right and left sides on the gill area and appears to have a distended belly. Here is a link to pictures.

Any ideas of what it might be?

Flukes?

Any other symptoms like flashing or heavy breathing? was it ever treated for flukes?
My kole tang does get some discoloration on it from swimming between the rocks and rubbing against them in the tank but in this case where its on both sides of the fish by the gills, could it from something irritating him in the gills and its scratching himself?
