- This is a quarantine tank that I've had set up for about 4 months.
2. If less than six months old, what is ammonia level?
-Ammonia level was zero.
3. What is SG of this aquarium? How measured?
- 1.026 using a refractometer.
4. When was the last fish added to this aquarium?
- I also had a royal gramma added to the tank around the same time.
5. Was it quarantined? If so, how? And how long? Was it prophylactically treated? How?
- Kole tang was in quarantine for about 11 days before it died. It was a new fish. I was treating with prazipro and cupramine.
6. If you are using a copper based medication, which one? How often do you measure level? When?
-I am using cupramine and checking levels with a Salifert copper test kit.
7. If you are using hyposalinity, how did you calibrate your refractometer?

8. Please describe in detail, the appearance of the fish? If there is one or more pimples, are they lumpy? What color?
- The fish's mouth at about day 9 started to look a little swollen on the inside. By day 10, it's mouth was so swollen, it couldn't even close it and the opening was smaller. There was swelling on the outside of the mouth along with a pinkish color at the top corners of the mouth. It's face got skinny probably because it couldn't eat anymore.
9. Please describe the behavior of the fish as best you can. Is it acting reclusive? Is it always up towards the top of the aquarium? Is it avoiding light? How active is the fish?
- The fish was acting great the first 8 days, eating and pecking every surface of the tank. It then slowly went into hiding and wasn't as hungry. It's breathing increase and eventually hid 100% of the time. It wouldn't eat anymore and then died.
10. Is the fish eating? What?
- The fish was eating mysis, nori, squid, shrimp, oyster, and stuff on tank surface. It eventually stopped eating.
