Kole Yellow Eye Tang Help!!

Hi everyone, i am having trouble with my tank. Last week my pearl scale butterflyfish died out of the blue. At first he was fine always swimming and the day before he died he just was hiding and breathing heavily in one spot. no signs of icy or anything so i was thinking maybe an internal parasite but for him to die that fast? well now my kole tang is showing the same signs except his color has faded. the only thing is that he is eating like a beast as usual. Any ideas?? please help!!
 
Also forgot to mention all of my parameters are immaculate including temperature and the kole tang readily eats a little bit of myso, seaweed sheets, and formula 1 flakes with garlic every day for a varied diet. Tank mates are a blue devil, Yellow tang, and a diamond watchman in a 55 Gallon
 
Reclusive behavior indicates high probability of velvet which is also consistent with quick demise.
 
Could be any number of things - flukes or intestinal parasites (should see stringy white feces), Velvet (sprinkles all over the body), Ich (some sprinkles on the body, fish rubs on rock/sand... may target the gills).

Can you see anything unusual on the body?
 
well they actually got along good i added them both at the same time and they are both juveniles as far as icy goes i don't see any signs on him right now nor have i in the past weeks but he does dive bomb into the sand. Velvet may be the problem but as far as the dust spots i can't tell because he is always hiding. In any case how is velvet cured?

There is a sticky on velvet, and my blog has a protocol as well.
