Large Lawnmower Blenny Died unexpectedly

I found my Blenny Dead next to my BTA this morning, I just added Black Diamond carbon for the first time last night. Everything else looks great! Cleaner shrimp molted last night, the tangs, urchin, bangaii's, clown's, are all doing great... I also added two very small fire fish last night that I haven't seen yet this morning but my main lights don't turn on till noon.
Do you think the carbon killed the blenny?
Do you think the small bulb tip anemone killed the blenny?
Blenny was in my tank for a little over a month
0 Amm
0 Nitrite
0 Nitrate
PH 8.0
starting dosing kalk in my ATO two days ago 1/2 tsp per gallon
tank is 120 gallon with a 40 gallon sump
 
You'll probably never know what killed the LMB. There is some evidence that carbon (dust, I believe) may be related to HLLE. But I can't imagine adding a good carbon would kill any fish.
 
Just trying to figure out if anything needs to be changed this is my first saltwater loss (other than a hermit and snails)
I want to get another one but this one looked very lively yesterday with a big belly happily munching algae off the glass.
Although when i bought it from the lfs it was in the hospital tank but this store seems good they refused to sell me a clown because it "wasen't ready"
 
To me, it almost sounds like it wasn't a random death. You stated you added carbon for the first time 1 day ago, and kalk 2 days ago. Something must have occurred in one of those things that killed him. Either bad kalk mix or bad carbon I would say. And I would watch your other tankmates VERY closely.

I am no expert, just my $.02!
