Lionfish dislocating his mouth?

My lionfish just tried to eat and when swallowing the food, he seemed to dislocated his mouth where the gills and the body meet. I saw a bunch of blood spew from him and he started moving eratically. I decided to take him out and put him out of his misery.

Has anyone seen this before? I know my tank has ich, and for the last few days his gills have looked kinda red and he hasn't been eating. The ich has only been in the tank for a few weeks and he hasn't been eating like normal for the last 1 1/2 months.

The reddened gills might have been a sign that something was wrong.

Any thoughts on this?
 
Ive not seen anything quite that dramatic, but yes, I had a lion die after 'dislocating' his jaw. He went for a piece of shrimp one day, and misjudged his distance apparently- rammed into the glass at full speed, and wasnt able to close his mouth again. I tried pushing it back (carefully), I even tried to force feed him- hopeful it would give him more time to recover for himself. Unfortunately nothing worked and he died a short time later. Ive heard of a number of similar cases.
