Looking for some more advice please?

Here is some picture of my butterfly fish.......anyone got any ideas whats wrong??? If your answer is improve water quality please give me some ideas. Ammonia and nitrite is at zero and nitrate is 5.0ppm or the 2nd color on my chart. Temp stays between 76.8-77.8 and salinity is always 1.0225-1.023.

What is history? How long? What source? Quarantined?

Only had fish 10 days, wasn't quarantined, 1st and only fish in tank after fishless cycle

It's hard to tell but is there mucus coming off the fish? Are his eyes cloudy? How is his eating and breathing? How big is the tank?

1 eye is cloudy and puffy, tank is 75 gallons, doesnt seem to have any mucus coming off. Eating and breathing seem normal.

That is typically from an injury. It looks like his color is faded on parts of his body, especially on the black stripes.

It looks like that to me too, what causes that or is that just his coloration?

It's normal coloration IME. It usually is associated with a disease or parasite.
