Maybe not ich? What to try?

I have a yellow tang and a spotted puffer in a 90 gal FO setup, about 5 1/2 weeks ago, I had a little breakout of ich. Did the hyposalinity thing, and all was looking good for about three weeks, then early this week spots back on the tang.
Now, these spots don't seem quite as white as what was on there before. And they are all on the body, none on any of the fins that I can see. I really can't tell if there is any discoloration on the puffer, because his skin is spotty and irregular anyway. In addition to the little pimply looking bumps on the tang, he also has some red streaks on his body and dorsal fin, starting from the bumps and kind of going out onto the fins. I am guessing that is some sort of inflammation/infection? :confused:
So, any suggestions? Does this sound like ich from hell, that won't die in 1.009 salinity, or is it something else that requires another form of treatment? Opinions on formalin baths, fresh dips, copper etc. I am thinking of setting up a 20 gal QT tank so I can better treat them and let the tank die off.
