Melanurus wrasse keep its mouth open wide

It's sluggish and cannot seem to eat. I believe my water is bad so I took it out and put it in a hospital tank. How do I treat and make sure it won't starve to death first? It stays on its side a lot.
 
With its mouth staying open I assume its having a hard time pushing water through its gills.

Please save everyone some time by posting your water parameters, temperature, etc.

Temp 83 Temp up recently due to summer heat but it does this every year w/o problem in past.

Have had the wrasse for 4 years. Other fish - yellow tang, coral beauty, pair of percs. Only inverts are snails and corals.
Tank is 10 yrs old, stable, 70 gal w/ 10g sump and 10g refug. Have added some corals recently but no new fish or inverts in a long time. Mouth has been open for a few weeks but was eating until recently, now mouth really open wide and fish not eating, acting lethargic, breathing fast. Seems like he cannot close the mouth at all. No visible lesions in or around mouth. No visible wounds. Gills not swollen.

Any Info on pH, Ammonia, Nitrite? These three mostly affect fish.
