MH Bulb Burnout

I have had my new tank up and running for a little over a month now. I have dual 250 w MH over the tank. I have standard M-58 probe start magnetic ballasts running 2 - 250 w 14k hamilton bulbs.

One of the bulbs would fire yesterday. I switched the bulbs out to see if it was the ballast or the bulb, and I had one bulb in both sockets and one bulb work in neither socket.

My question is, does this happen often (A bad bulb), and if not, could my wiring have something to do with it. The bulbs have both fired fine for over a month now.
 
One bulb doesn't work in either socket while the other does? Bad bulb. No it doesn't happen often that I've heard of.
 
not common at all but it happens, contact your bulb source and see if they will warranty the thing. Wiring size and type can cause no starts (I found this out with a 150W HQI)

Are you using the supplied wiring or did you improvise some?
 
I hadn't either, but I didn't know why one would just stop after a month with no sign of damage and the other one works in both sockets.
