so I would need a ballast and a pendant and then a bulb?

so the icecap, one of the pendants, and the ushio DE bulb are what i would need for a full setup?

That would be all you need, but the icecap is already at my house.

BTW, I had one of those same pendants with a ushio bulb over my tank for a couple weeks when one of my ballasts died, and I needed to borrow Chad's lights for awhile. It looked awesome.
