I got 2 very small mushrooms a few days ago from a trusted LFS. They are about 1/4 inch in diameter. However, I noticed yesterday that one had something like small white worms on it. Is this normal? That coral is not doing so well.

Are you sure they are worms? You can see them moving around?
Lots of times Mushrooms will spill out their guts as a defensive measure. It looks likes stings coming out of it's mouth.
There are flatworms that might eat mushrooms. You can do a search for flatworms for pictures.
If the mushrooms is in bad condition and decaying, them many worms that are normally reef safe may be taking advantage of the dead tissue..
