my puffer needs help!!!

Problems more of needing two people. The gf holds him, in a wet wash cloth, and I have to b able to see his mouth to get it open (with a Popsicle stick) then use the other hand to get the syringe/tubing in to get it down his throat.
 
I see. Have you tried putting your thumb on one side of the jaw and your forefinger on the other and gently squeezing to see if that will open the mouth instead of the Popsicle stick? That's what I did and it worked pretty well. Smaller puffer though but the idea is the same. That allows you to hold it cupped in one hand (your palm under the belly) and use those fingers to manipulate the mouth leaving the right hand free to feed.
 
Update: jaw is a lot easier to open when force feeding. However, eye is not getting any better with erythromycin, may in fact b a parasite so thinking of giving a freshwater bath? Or using a medicine for parasites. Any suggestions? Got some more pics I'll post in a sec.

Honestly I would take him to the vet, if they think it's bacterial they will prescribe the appropriate antibiotics

I agree with the above.

I'm leaning toward that or parasite. My initial thought was perhaps some burst blood vessels due to being handled. This is pretty common when grabbing hold of a fish. But this looks more serious than that.
