My Toadstool Dieing?

I had another post where he was acting odd, he still opens up everyday but noticed the head seemed to be getting smaller. He's only attached to a small piece of rubble so I moved him closer to the glass and it looks like he's peeling and has yellow blotches if you can see in the picture. What should I do, leave him be?
 
How long have you had him? Leathers molt, normal process and especially if they have been handled a lot, different water perimeters, acclimation, etc.... It's a normal process, let him be. Can some times take a week for them to go through the process. When it's complete a film will peel off them and they'll be back to their glory.
 
I've had him at least 6 months. Just wanted to make sure it isn't rot or Id frag him. The end attached to the rubble is very shriveled up almost like its rotting away but I'm not sure

Well bad news the whole stalk tonight looked bad, and it wasn't the best smelling thing, it wasn't unbearable but it didn't smell good. When I pulled it out the bottom of the stalk where it was attached was withered and yellow and it went all the way to the head. The head still had polyp extension and looked fine. I cut that into 3 pieces and tied them to some rubble and placed them back in the tank. We will see how they look tomorrow.
 
Update: the 3 pieces all had polyp extension today, the one piece looks like it has a little of the yellow stuff on it so I might have to remove it but the other two are looking okay
 
Next update, all 3 pieces have had polyp extension, one came untied from its piece of rubble and I fixed that. Bad news 2 of the pieces have a yellowish "coating" over where they were cut, it scrapes off but I don't know if they are starting to rot too or not? They just smell "fishy" but what should I do, is the yellow on the cut parts normal?
