Naso lituratus with ich advice

I've had a 6" Naso in QT for 2 weeks. Week one was just getting him to eat. Fortunately he was / still is super fat. I had a tough time getting him eating. He will eat the odd pellet and shredded red nori. Oddly enough seems to prefer it soaked in selcon. I noticed a few black spots on him and but not really any ich. Started treatment with prazipro 3 days ago. Last night I noticed a few spots on his fins but still unsure if ich. Today he is COVERED.

I have seachem cupramine and copper test kits and have treated fish this way before. I have never done the tank transfer method. I have a 20 gallon tank I could use for second tank but would have to get another heater and maybe hob filter or something to go with it. The QT tank is a standard 55 gallon. Unfortunately I don't have any fresh saltwater ready but I could have 20 gallons today / 50 tomorrow. Do I have to do a large water change first before treating with cupramine? Should I just go with the tank transfer method instead? What alarms me is how fast he went from a few spots that might be ich to covered. Since I added prazi he has lost his appetite which just as easily could be the ich and not the prazi which is what I assumed. I have no concerns over him starving since he is still fat, but I am concerned with how fast ich is covering him. The pic is not the best glass is dirty, hob filter puts out a lot of microbubbles but you can see he has it pretty bad. Normally I just have a dim non aquarium led light over the tank but put a couple of t5's over for the picture and to examine him better.

Trying to decide which course of action will give him the quickest relief.

Decided to go with copper as it's less labor intensive. I have some long days at work and don't want to be caught too tired to do a proper sterilization and transfer.

Will post an update in a few days or if anything changes.
 
He's looking better today with less spots but not eating. I am only going to try and feed him every other day to avoid polluting the water. That being said I need to go buy more fish food and if the store has live brine I will try that.

I don't think I should be overly concerned about the eating as the ich is obviously bothering him. He's still swimming around and fat so can obviously go a while without eating.
