Naso Tang help needed

Okay I have asked this before, my Naso has spots/blotches inside which I was told were due to stress, but he has had them since I have got him about 3 weeks now, I have now noticed something else as well, he eats the most out of all my fish but he is getting skinnier his stomach is looking sucked in and I can see the line sort of bone on him, I called the place I got him and they said it was worms and that they will give me something to soak pellets in that will gett rid of them, can someone chime in on this before I do this, like what type and name of stuff should I be using?

how often do you do the water changes? it might be becuase of the bad quality of the water

weekly, sometimes biweekly, water parameters fine I check them frequently also using Kent RO/DI with Reefcrystals, as said all others are fine
