Looks like an injury to the mouth with a bacterial infection. This likely happened during capture or transport. It wasn't anything you did. Mayber send the pics to the seller and see if they'll make it right. Pretty fish nonetheless. Sorry it didn't make it.

Sorry to hear about your wrasse. These mouth injuries are pretty common with these wrasses during transport. They are very skittish and "smash" their mouths on the inside of the bag so much it causes injury.
ohh right. I'm slow. I already contacted the supplier as soon as I pulled it. I'm now thinking of another nice hawaiian fish I could take as replacement
