Orange tile sea star

I placed an Orange Tile Sea star in my 65 gallon reef aquarium. I acclimated it for about 45 minutes. The first couple of hours it moved around on the glass, then I found it upside down in the sand. I right sided it up and it hasn't moved in a day or two. My parameters are PH 7.2 Alk 4.5 Calcium 450 Nitrates 0 Nitrites 0 Ammonia 0 Salinity 1.025. Other fish I have are....2 yellow tail damsels, sand sifter gobie, 2 clowns, blue regal tang, yellow tang, bloodfire shrimp and a red serpent sea star. Any clue as to why it has died?:spin2:
 
I'd say you may have acclimated it to quikly. urchins and sea stars, that is to say echinoderms, must be acclimated very slowly ,1 HR. min. I take at least 1.5 HR. They are really sensitive to salinity and PH changes.
