Paraguard Death

Wondering if anyone has had trouble using Paraguard. I have had two tangs succumb in QT this week. Both were 2-2.5 inches. Bought them from the LFS. The first one, which was the largest of the two, I treated with a full dose of 10 ml in a 20 gallon tank. It didn't make it through the night (treated it around 5 pm). The replacement, I treated on the first day with 1/2 a dose of 5ml in 20 gallons. The second day I treated with full dose of 10 ml in 20 gallons. It didn't make it 8 hours after treating it around 6 am. I know these fish had ich as I could see it. Now I am wondering just how much or what else they had. I know the salinity at the LFS was 1.021, which I matched in my QT.. Wondering just how much stuff the LFS was trying to fight off.

Thoughts?
 
Hard to say whether it was the Paraguard or something else. FWIW, Paraguard isn't a good option for ich, as it's basically "designer formalin" (it's glutaraldehyde + malachite green). It might be good as a dip or possibly against Brooklynella, but there are better options for treating ich (I prefer TTM myself).

Fishvet 37% formalin is a better dip than Paraguard IMO for velvet and Brook, and TTM is easy and very effective for ich.
