Problem with Tang

Can you help me identify what is wrong with my Tang and how to treat?

Looks like HLLE but a better picture would do a lot more.

Head and Lateral Line Erosion goes away with good nutrition. What are you feeding?
 
Fish is new in QT. It has been there for about 4 1/2 weeks now. He was in cupramine at the fish store and appeared to get ich within 2-3 days in QT at my house. Following the LFS direction I treated him for 3 weeks keeping my copper levels between .3 and .5 (seachem copper test). The fish is very shy and hides alot. I am having a hard time getting a better picture than the one I posted. Once I get a better shot, I will post. The tang seems to be eating ok and acting normally. He just looks really ugly. I feed omega one marine flakes, New life spectrum pellets every day. I add Hikari seaweed extreme, frozen misis shrimp, and frozen omega seaweed once a week. I also put a small sheet of dried seaweed rubber banded to a rock 2 times per week. I have seen the tang eat everything except the misis shrimp.
 
LFS running copper mask certain parasites. 3 weeks is not sufficient even if you maintained therapeutic levels and copper is bad for fish. Look at tank transfer for removal of ich. Reclusive behavior may indicate a more serious parasite. Describe the appearance.
 
The spots are pink and are mostly located around and between the eyes. There is one spot high on its back. They look like a bare spots (scales missing) or sores and seem somewhat indented (like a dimple on a golf ball). There is also discoloration on the gill covers. The discoloration is white and is not all indented. The white areas really look more like a bleached out stripe. Fins and tail look good. No rips or deterioration.
