Prophylactic treatment--do you?

I've not really had any problems using careful QT for observation and no pre-treatment of fish unless they show themselves to be sick.

But I'm wondering--anyone care to chime in on the topic of prophylatic treatment of all new fish in your QT? Pros and cons? Or cons/likelihood of problem in standard QT 4-5 weeks if fish are asymptomatic? What is the risk to a healthy fish of prophylactic treatment, AND--what do you use/treat for?

Context is a flame from Marshall Islands, 2 weeks in my 20g long QT, eating well, so far so good, no symptoms at all.
Thanks.

I always use hyposalinity. It does more than just help fish with ich. Prazaquantel is safe so I don't mind using it routinely, but that is the only drug I would use unless you see symptoms of some sickness.
