Pyramid snail question

I have a couple questions regarding these little pests. First, how does it actually destroy a clam? From all I've read it seems like these little suckers are on the shell and not on the mantle or inside (reason for brushing the shell). Do they get the clams from underneath and to the byssal gland? The reason i ask is because I believe i have seen my first pyramid tonight, but it's right on the mantle. So if that's the case, how do i scrape it off from there? Use tweezers maybe? There is only one that I can see so maybe I am overreacting and that it is something else. I'll try to take a pic tomorrow, maybe that would help also.

most of the time they feed in the dark. they go to the edge of the shell, up to the under side of the mantle or along the byssal opening and use there proboscis to peirce the mantle and feed off the blood
