QT set up questions

I'm getting ready to set up a QT tank for new fish arrivals from LFS. What will I need?

What size?
Filter type?
Carbon through reactor?
Lighting
UV?
Heater?
No gravel?
Plumbing fittings for shelter?

Any pictures of exiting QTs?

Size should be determined by the type of fish you plan to keep. A 15-20 gallon generally works well.
A heater, small sponge filter or hang on filter with cultured media. A small powerhead for flow (not too much though).
A couple/few pieces of 2-3 inch pvc pipe cut to varying lengths for shelter. Regular strip light or even ambient room lighting to keep stress at a minimum. No substrate or live rock in the event you need to medicate or treat with hyposalinity. Keep it simple;

What if the LR is unwanted rubble. Mine has some aptasia, and I want to use it for biofiltration in a Hypo QT. Will that help alongside sponge?
