Question on wiring a ICECAP 250 with EL Ballast

I'm trying to figure out how to wire an ICE CAP Ballast, in the picture everything is pretty self explanatory except for a couple of things.

1) Do you connect all the "D's" together than ground them to the box or just connect the one "D" on the left hand side to the box and connect the two "D's" on the Right hand side to each other?

2) where the ground symbol is on the ballast itself do you just ground that to the box or does it have to come out of the box and hook onto the light fixture somewhere.

Sorry for the basic questions I'm not very knowledgeable when it comes to this type stuff.

If you tie all of your grounds in together, that will ground out the ballast, lamp, and wall plug. HTH
