Random polyp death on blastomussa? Ideas?

I've had this colony for 7-8 months and it has been growing, but once or twice a month a polyp/head dies. I can't figure out if its predators, normal behavior, or something else. I fragged it a few months ago and the frag isn't doing this . . . . but it only has 11 polyps.

Ideas? Really small 'something' I should be looking for?

Tank params:

Salinity: 1.025
Nitrate: < 5ppm
Nitrite/Ammonia: 0
Phosphates: undetectable
80F
Medium/low flow
Medium light
260 watt PC lighting

You can search for a parasite- nudibranch or similar. If it is a nudi it is likely to only eat B. Merletti. I don't think this is normal.
