to me it looks kinda bleached. it is hard to tell from the pic. what lights are you running? where is it located in the tank(high, mid, low)? i would probably move it to a lower light area and try to get it to eat some mysis or something else meaty.
hows the flow, mine did better when I put it in a lower flow area and it did really well when I stopped feeding it, believe it or not. I also think that they tend to open up a bit more on the sand as opposed to the rocks. good luck, corals can come back from almost anything in my eperience if your patient.
I'd bet on the flame angel. Just pulled mine for the same reason. It caused no problems for over two years. Then one day I caught it nip an acan. From that point on, it was a menace. It started picking at every fleshy coral in the tank and actually got super aggressive with the other fish. Certainly test everything else, but keep a close eye on that flame.
Are the clown fish using that coral for a surrogate anemone? I have a similar coral and my clown has adopted it as his home. I've had the coral for years and it has always thrived but it has recently started to decline. Everything else in the tank is fine and I am convinced that the clown is irritating it in some way and preventing it from thriving. I might be wrong (that did happen once before, but then it turned out I was right ), but that is my leading theory at present. Might have to yank it and see if it does better somewhere else, or somehow get the clown to stay off it (no idea how to do that!).

about 3 or 4 years ago, my open brain bleached out completely and looked alot worst than yours due to my own carelessness but I fixed my water problem and feed it shrimp shavings and cyclop-eeze twice a week and after a few months it was as good as new. I still have him.
as long as its still eating then you should be good to go...
be sure to check all your water parameters... alk, ph, salinity, etc etc... oftentimes its best to cross the tests at the lfs due to several reasons.
