Sick clown fish, not sure how to proceed

I have a clown fish that has been in the tank for at least 2 years. On the 11th I noticed he wasn't really eating. We were feeding more frequently so I just assumed he may not be hungry. By the weekend he was being a very picky eater. He was sucking in food (Rod's food) but spitting it right back out. I did see him eat some very small pieces though. The following week, on the 17th or 18th, he was in rough shape. He was very lethargic, he was doing what appeared to be slow front flips at the bottom of the tank.

So I watched him for a while and noticed something poking out of his anus, It didn't look like poop or a worm. I started looking up possible issues and by time I came back it was gone but I saw something very similar looking in the sand. I sucked it up and took a closer look and it appeared to be a very small piece of a shell. I can't be 100% sure since I didn't seem him poop it out.

The next day he was much more energetic, he was eating(Formula Two pellets) a bit but still not normal. I watched him throughout the day and his poop was white and stringy but he was swimming around fine. Over this past weekend he is doing MUCH better. He is eating like crazy but he still has stringy poop, not white, and he is very energetic.

Also last month we had a Tribal Blenny die. He got really skinny and stopped eating. We just assumed it was because we took his primary food source away which at the time was green hair algae, he graze don this frequently. Over the course of a week 75% of it was gone and we could not get him to eat algae sheets. Now I am wondering if it was something else.

So now to my question... I do have a quarantine tank setup and I was wondering if I should treat the clown with PraziPro. Also should I treat the other fish in the tank since we lost a fish previously?

Other fish include a Watchman Goby, another Clown, and a Fire Hawkfish all of which have been in the tank for over a year.

All the other fish are eating fine and appear healthy. Thanks in advance for any help or suggestions.
 
I'd treat all the fish with PP. (I've treated all new fish with it for years) PP is probably the safest med we use and can even be used in a reef tank. It will kill feather dusters, though.
 
Thanks for the quick response! I kind of expected I would need to treat all of them but I didn't want to if it wasn't needed or could cause more harm than good.

One of the few meds that does what it says, actually works, and doesn't have a downside. Your fish won't even know its there.
