Sick Tang - maybe hole in the head

I had a tang in a copper treatment several months ago for Ich. He seemed to lose a few scales. I lowered the copper level and he seemed to so ok.

Since he has never gotten the scales back and I am afraid he might have some other ailment. He acts fine and eats. No other fish have any issues.

Any thoughts? Sorry for the bad pictures. He does not like to pose.
