Soft coral disintegrating, is it normal??

Hello,

I just started adding corals and a soft coral is disintegrating like wet paper, what should I do?

Also, another coral keep secreting mucus like stuff, what does it mean?

Please help and thank you.
 
My tank is 125 gallons, I have every single piece of equipment, skimmer, UV, power heads, T5 lighting, Nitrate control equipment but I don't have a refugium.

Parameters for the standard stuff read fine, but I use a standard testing kit. Calcium I don't test but I add it without failure per the instructions on the bottle. I've only been keep corals for 3 weeks.

Type of coral I could not tell you because the pet stores confused me, they each name everything differently. The coral with the mucus is like a plate and with light little fan like heads come out and extend towards the light, it's green. The disintegrating one is a on a branch type piece of rock and with light each polyp grows wo make what seems to be on large coliflower. The color is brownish.
 
I am a big believer in don't add what you can't (or don't) test for! To help we will need exact numbers. ammonia, nitrate, pH, calcium, alk, temp, and water change habit as well as what salt brand you use. How long has the tank been setup for?? What kind of fish?
