SPS Id and Growth or Death?

I'm pretty new to reef keeping. Can anybody Id this guy and let me know if the white tips are growth or bleaching? If bleaching what do I need to do besides moving the piece?

It looks like your coral is bleaching. Please take some test and tell us what is your nitrate and phosphate

Here are the fresh Params (just taken):

Salinity 1.026
pH 8.2
Calcium 520
KH 7*
Phosphate Less than .25 PPM
Nitrate 20 PPM

I will be doing a water change today.

your phosphates are way too high in my opinion for sps, as well with nitrate. all is too low for sps and cal is high.

I am getting a skimmer this week (nitrate reduction) and just added cheato to try and reduce the phosphates. Any tips on raising alk? I can lower the amount of calcium added to the system.
