This is killing me, and my trigger

Fish is in hypo for ich, it's been 5 weeks now. No visible signs of ich. The last two weeks have been a slow decline. He started out not acting as active, then developing a bulge in his stomach, I started a treatment of maracyn and maracyn two. I saw no change, he got worse. Started listing, and losing his ballance. Now he's laying on his side, and only moving when I feed. His eyes are clear, and he is alert, he responds when I go over to the tank, but he just can swim, or it tires him out. Is there anything I can do at this point? I hate this, if he wasn't so responsive it wouldn't be so bad.

Tank in hypo 11ppt
Ammonia 0
Nitrite 5-15
Nitrate 5-10

I know this is not ideal, but I am doing my best. 50% water changes everyday, using prime, and algone. Other fish appear to be doing ok.

What are using to measure the salinity with? Why are you still treating with hypo at 5 weeks when the fish has no signs of ich? What is the pH? Do you store all of your foods including flake in the freezer? You do know that the pH must be monitored daily in hypo right?

Using a refractometer, monitored everyday, I was very careful to make sure my water prams matched at water changes. I started raising the salinity at the start of the 5th week. So they were in hypo 4 weeks, and are on there way out. This started before the change in salinity, and I'm taking it slow 1-2ppt every couple of days. PH between 8.0-8.2. I feed mostly frozen food, but I do supplement with pellets and sinking disks. I do not store these in the freezer. I also add selcon to the food, and I use a variety of frozen foods.
