1. How old is this aquarium?
I just received it from my cousin Sunday... so 7 days ago. My cousin has had it for over a year. We bagged the water, fish and live? rock and sand separately and drove it all to my house and reassembled everything. (over and hour car ride and an all day project to reassemble everything)
2. If less than six months old, what is ammonia level?
3. What is SG of this aquarium? How measured?
I don't know what SG is...
4. When was the last fish added to this aquarium?
I don't know. I just moved the entire tank 7 days ago into my house.
5. Was it quarantined?
No. I just got the tank so I don't even have quarantine tank set up/available! =(
6. If you are using a copper based medication, which one?
I'm not using anything yet because I'm not sure what is wrong.
7. If you are using hyposalinity, how did you calibrate your refractometer?
???
8. Please describe in detail, the appearance of the fish?
Black and white stripes... I think he/she's a three stripe damsel fish. Its about 3x3inches.

9. Its gill covers seem always open? and it has swollen gills? bulging outward and it is breathing rapidly. But it seems/acts okay, eats and swims around as much as the rest of other fish. Its fins are not clamped, he is not scratching on things, there are no missing scales, discoloration or visible fuzz or parasite hanging on its body. It is not acting reclusive? Does not hang around at the top nor bottom of the aquarium. I don't think it's avoiding light, only avoiding the bigger meaner fish.
and none of the other fish seem sick at all.

I've been feeding the bank 1/2 tsp of Tetra Color Tropical granules.
I've been researching online but I'm not sure what it is. Is it flukes? Was he stressed/hurt by the move??
