Toadstool Question

I was wondering if anyone had problems with Kent Essential Elements. I had a Toadstool that was about 12" and died shortly after I added this. I thought nothing of it becaust all my mushrooms, polyps, zoa's, and kenya trees look great if not better. I took a frag out of my other tank that I got off my original toadstool and it was fine. I went and added the Kent stuff again and my little toadstool wont open up and isn't looking to well I was just seeing if anyone had any ideas. Should I just not use anything, and has anyone else ran across this problem. Here's pics of before and after I added it.

i had some issues with my toadstool not too long ago...turns out mushrooms that grew onto its rock were stinging it. i separated and it came back to good health within a few weeks. were there any other corals near it that could harm it?
