Torch Coral - fast deterioration

so my torch coral retracted a lot last night and i was a little worried before i went out. when i got back one head looked like it had melted away almost completely and the other was looking a little better but retracted also. i moved it into the light to investigate no brown slime visible. but as i moved it the flesh from the first head just tore out in the medium flow! i have never seen this happen.
It pretty much deteriorated within a couple of hours, no brown slime!

now im sure you are wondering if my tank is doing okay.
i did have a shrimp die the day before but he seemed rather stressed than anything, as for my sps, they are doing great all polyps extended. all water parameters are perfect, and the rest of the lps are doing great too!

i would have posted one but i cannot as it flushed out of the calcium body when i inspected it. would just like to know if anyone else has had this happen and if anyone knows why this could happen

Polyp bail-out. It happens when the polyp is unhappy for an extended period. Were any of its neighbors touching it? If your other parameters were truly perfect, unruly neighbors would be the only other cause, unless it wasn't getting light or food.
 
See that's what i dont understand. i target feed and the next thing that could possible touch it is 4 inches away. lighting is perfect and it was fully extended about two days before. is it possible for the bailed out polyp able to grow again i think i see it in the back of the tank where i cant get to it. but it can get food and light there ...

Supposedly that is the coral's last ditch effort to find a new place to live where it will be happy. It is rare that it will survive, but it IS possible. Though I have no first-hand experience, I have read about it. So take it for what it's worth.
 
My torch coral lost a head because the other heads had grown faster and blocked it from getting light. So, maybe check placement?
 
I'll put it in a glass shot glass if I can get it out.

As for placement it is a frag tank with frag racks.

If you feed them to much sometimes the polyp outgrows the corralite or base.it has also been related to mag fluctuations
