Tunze 6105 Fish Died Swam In I Think

I woke up this morning to find one of my small Ocellaris Clownfish to a Tunze 6105 circulating pump. The fish looks to have swam into the front of the Tunze grill and was wedged between two of the square grids on the face of the pump. Has anyone else experienced such a loss like this? I can't see another way the fish became lodged in the pump. The front cover was firmly on the pump.

The Clownfish looked healthy, no issues at all. I was running the 6105 pump in the pulse mode. I think the fish tried to swim into the surge. I was hoping someone had a preventative measure for this.

Thanks for any help with this, its really frustrating this is one of two Ocellaris Clowns in my DT and my first fish overall in my tank.
 
We do everything possible to prevent these sorts of incidents by including the fish care function where an off pump twitches and the protective grid, I would think this would be very unusual and rare but certainly not impossible if a fish is small enough.
 
I don't think there's a large enough opening at the back or sides of the pump for the fish to get sucked into, it was a small Clownfish maybe 1 ½ inches long. Only the cover of the pump has big enough square openings for the fish to fit into. If the fish was not healthy, I would assume it would have a very hard time swimming up-stream and into the square openings. I don't believe there is any suction at the edges of the Tunze 6105 pump front cover to pull a healthy or sick fish into, I could be wrong however on that. A sick or weak fish could easily become stuck to the pumps intake area which comprises most of the pumps exterior area. The pump is in the pulse mode running from 100% down to about 40%.

A bit of other info, the fish had no marks other than where it was lodged in the pumps square front grill. If the fish had been sucked into the pump from the sides or back one would think the fish would have marks from the fish being sucked through the running impeller.

I had someone suggest the fish was in a weakened state, but I just don't think so. I was hoping someone had a way of covering the pumps in a way to prevent this, especially if had happened a few times to others and they had some experience with this type of thing.
