Was this Ich?

I bought a Hippo Tang (and also a Maroon Clown) yesterday and brought it home and put them in the QT. I acclimated them for about 3+ hours (drip method) and all the water params seem good to go (0/0/0). Salinity around 1.023. I don't remember there being any spots on her when I brought her home, but I got home this afternoon and she was laying in a corner with spots all over her, breathing heavily. I immediately moved her into a 1gal tank (she's only about 1", if that) and waited to see if she got any better. Well, she didn't and she died just a few moments ago, around 24 hours after bringing her home.

EDIT: Also, I think I should add that the fish was scratching on some rock off-and-on since I brought I home.

Can someone tell me what, if anything, I did wrong? Is it possible that I did something to create THIS much Ich in under 24-hours? Was it likely that the Ich was already there before I brought it home? Should I ask the LFS for some credit? Gah, so many questions...thanks for the help.

Ich does not kill that fast. The disease that looks like Ich, but is MUCH more deadly is called velvet. This is a rapidly progressing disease which starts off at the gills and so shows initially as respiratory distress but will then spread out across the skin.
Signs include respiratory distress, skin rubbing and erratic swimming. Without treatment death can occur within two days of respiratory signs.

That would be my guess if it killed that fast.
