wavebox problems

ok i took it apart and cleaned it.. i took it apart to check it... it will run for a week and start beeping... then it will run for a day and start beeping.. then beep 12 times then start running... i need to send it in to get checked out... where do i send it and what do i send with it.. i dont have to send the actual plastic box... just the driver , pump and the controller.right....thanks
