What happened to my nem?

I have (had?) a large beautiful Entacmaea quadricolor that has been doing amazingly well. Now I have what is shown in the photo and can't find anything else. That blob is about 1.5" by 1". Maybe more of him is hiding in the live rock?

I don't know what happened, the edge of it looks almost torn like it was cut with a dull knife.

Is it likely that it got caught in my powerhead? If so, what should I do, will it poison the tank? I have carbon in the sump.

My tank is about 10 months old. My specs:
Ammonia 0
Nitrate 0
Phosphate 0
Calcium 440
Alkalinity 11.8
Magnesium 1380
Salinity 1.023
PH 8.04
Temperature 78.4

Over the last 3 days I have done one water change each day. The first day I changed 10%, the second day 8%, and the third day 8%.

Through each water change I have been increasing the salinity a little so it is now up to 1.024.

That's the background, but what should I do right now? Water changes, carbon?

Based on the photo it looks dead. I would remove it, do a large water change, and run fresh carbon.

Only you'll know if it had a run in with a powerhead. BTAs and powerheads never play nice.

How big is the tank and what are the inhabitants?
 
Thanks for the reply D-Nak. I don't know how to tell if it hit the powerhead or if something else happened. The water is clear and no pieces floating around. Odd that I can't find more of the nem though. I'll do a big WC, remove the corpse, and run fresh carbon. Sad day.

The tank is 90 gallon, I have a Green Chromis, Bangai Cardinal, Skunk Cleaner, 2 Sexy Shrimp, Blue Leg Hermits, Snail clean up crew, large mix of LPS/SPS coral.

Make sure its dead, do you gave a QT tank? I have pulled my nem out of my Hydor Powerhead and looked bad for a few days but soon it looked great again
 
Kissman, glad your nem survived! Yes, I do have a QT, I was thinking about putting it in there, couldn't hurt! I see you're in Waynesboro, VA. I lived in Rapidan for a number of years (between Cupeper and Orange). Beautiful part of the country!
 
Yes as long as there is nothing in QT to kill him or futher him off. Rapidan is nice, I love this area. You moved kinda far! LOL! I knew it was possible that the nem would walk till he found his happy spot. Been in the same spot for over a year. Two weeks ago I added about 12 SPS and what do you know he has moved 3" to the left. WTH. Hope thats all he does. I put him in before adding any coral. Good luck
 
Is it possible for a large RBTA (spread of 5-6") to become a 1.5x1" blob? I looked at the foot and it seems complete.
