Aggreed, Good Husbandry goes a long way in preventing it's occurence in the first place. And Yes i have heard of people having these issues when starting to add a carbon source but like you said it may just be the trigger not the cause.

I think it takes a while for the bacteria strains to get sorted out. When first starting you probably kill off some of the existing that don't like the new carbon source while the ones that do are building up. This period gives rise to the bad/cyno bacteria.
