What is this? Hole in the head???

I was wondering what you all thought this was. I hope it's not hole in the head. it is right where his left nostril is at. The area is larger than the right. It is about a 4 to 4.5 inch emperor in my 180 reef. Water quality is great and he is fed Spectrum, PE Mysis, Formula 2, and ocean nutrition marine algae sheet w/ garlic. He also gets plenty of light. I do run carbon. Any advice. You will see the small lesion to the right of his eye. Could maybe be a larger nostril? From the other picture he has great color and no sign of lateral line.

Hi Colgan. From those photos it's hard to see anything wrong with your angel. We will need better photos, close up on the face and as in-focus as possible to offer any help.

How long have you had the fish? If there is some sort of lesion keep an eye on it and make sure it isn't getting worse.
