what is wrong with my clownfish?

Feed him food soaked in garlic while in QT and make sure he is not stressed. He should be able to fight off any further battles with ich if he is happy and healthy after treatment. While garlic is by no means a treatment there are some people who swear it helps.
 
if it is ich you will have to remove all fish from your main tank for 72 days or it will remain in your tank sorry to hear your fish are sick

This is a matter of opinion... IME, in good water conditions, with well fed, happy fish, most can resist and fight ich off (many times without ever showing symptoms)... however, this guy has it pretty bad as previously mentioned, you should probably QT and treat him. Is he new?
 
A great method that I used to take the ick of my porcupine puffer was to quarantine him, crank up the heat in the tank to say 85 and put in a pump to create bubbles to get plenty of oxygen in the tank. Although some may not agree with the bubble method due to it can harm the fish by getting caught in there gills, it was never a problem for me. If you do not have a QT i would suggest getting a UV sterilizer to kill of any infection within the tank so it will not get on any other fish.

Everyone please know that I am far from an "expert" on this topic, so I am just speaking from an experience I had with one of my clown fish. I somehow purchased two of the most nervous and unsettled clownfish on the planet. They had been in my tank for about a month when I added a goby, can't recall exactly what kind he is, but he's striped, he is not the one that usually pairs with a pistol shrimp. Anyway, one of my clownfish became incredibly agitated that the goby was taking up residence near where he liked to hang out. He became very distressed and looked to have ich. As soon as I saw it, I put him in a quarantine tank, but he ended up passing away anyway. I totally freaked out thinking my tank was finished, but oddly enough the goby and the other clown fish did ok.
Sorry to hear about your sicky! I actually get pretty attached to my fish and I hate to lose one.
 
Looks to be more than just ich. The eyes are clouded over which could be bacterial or flatworms. Could also be marine velvet with ich. Have to do some research.
