What's happening to my coral?!

Went to do a daily walk by the tank and noticed my torch looked REALLY unhappy. What the heck happened?! About a month back I lost a hammer coral due to brown jelly, but I don't see any brown jelly on it! Other than that, I'm really at a loss.

I checked all params last night and all levels are fine. Nothing out of the ordinary or new other than adding micro algae to my sump recently. I'm keeping an eye out. The other two heads seem fine and are opened up today like normal. Could one of my hermits have walked by and cut it with its shell? One he a fairly sharp shell? That's all I can't think of. So confused and sad. :(

Sde1500 - I've had the coral for almost two years and it's always been on the sand bed. My clowns are hosting it too. That's good to know that though because I've researched all my corals and try to find the best placement but got conflicting info on that one. Plus, it was on the sand bed at the lfs.

It would do much better attached to a rock. I would bet the sand is bothering the polyps, along with the clowns. A little more light for the head that is receding likely will help too. I lose some heads on my torch occasionally, as larger heads above it start to shade them more.
