What's wrong with my clown?

Below is a good picture I got of the clown that we have and we are unsure of what could have caused the damage. Also we do notice at times little white stringy worm things on it once in a while but they are very hard to see. Any opinions/advice would be welcome.

Not sure post a link in the disease forum. If is a worm you need to treat it in a QT tank.
