whats wrong with this clown ? please help !!!

the pair of clown was introduced in the tank a week back 2 days back i found one clown dead and the other clown was getting some spots like this

please identify what is this and remedy for this.

Hard to say. The things around the mouth could be a parasitic isopod/copepod of some kind. Or it could be a growth. It also looks like you have something going on around the fish's dorsal fin. I can't tell if those are white sprinkles (Ich, Velvet) or possibly Brook.

If it's a parasitic isopod, you can either try to manually remove it with tweezers or perform a f/w dip to see if it will fall off. But to be frank with you, that clown doesn't look good. I'd say his prognosis is bleak.
