Why are my wrasses disappearing?

You can find a bit more background and detail in my tank build thread but here's the skinny...

About three months ago I bought a very healthy mystery wrasse from a guy that was tearing down his tank. It was doing great for over a month and then just disappeared. About a month ago I added a McCoskers from DD, it was also doing great but just up and disappeared about two days ago. Any thoughts?

Here's a pic of the tank. Its very open and I wonder if there are not enough good hiding spots???

I'm currently keeping two other wrasses, a Red Velvet I've had for close to a year and a Red-Headed Solon I added the same time as the McCoskers. The PBT, Sailfin, and Falco Hawk were all a bit agressive towards the McCoskers when first introduced but it seemed like they had it all worked out weeks ago.

I'm open to any ideas or suggestions

What is it covered with? Eggcrate? If so, that's not good enough, and wrasses can easily make it through.
 
The tank has a full hood and any gaps are filled. I'm also running a filter sock on the drain and I already checked it.
 
my falco scared 1 fairy wrasse and 1 hogfish right out the back 4" of my canopy....the wrasse hid for a few days when i first got him (completely disappeared)
 
The fish did not jump out. I've checked and check again, there is no where it could have got through, everything is sealed up tight.

I believe both fish died abruptly and I'm wondering what might be the cause???
 
Same thing happened to me with a blue hooded wrasse, its very odd and i have a similar set up as far as socks and screens. Wierd thing is i never even found the fish as i assume the cuc got it...
 
Treat all wrasses with prazipro. They usually have internal parasites. Did they appear thin before they disappeared?
 
Both fish were fat and happy when they disappeared. Plus the mystery wrasse came from an established system. If it were internal parasites would they have shown symptoms?
 
Any large crabs? Just talked to a guy that had a calico crab kill a feminus wrasse. Didn't know the crab was in there because his harlequin tusk kept it in the rocks, at least we think.
