why do my octos keep dieing

I have a 210 gallon reef system with a 55 gal refugium and I just set up a 20 gal octo tank, i acclimated the octo to my reef tank while my 20 gal cycles and tossed him into the refugium. It's four hours later and he s dead. I tested all my parameters and everything is fine. this is the second octo in five days that this happened to. any ideas what it could be?:(
 
If salinity and temperature are properly acclimated and ammonia and nitrite are low, the most likely other factor is oxygen titer. A more unlikely cause, but one that can happen is that something toxic has gotten into the system. The most common would be copper which can enter the system through the treatment of fish or metal fittings.

There are test kits available for around $10. Dissolved oxygen can change dramatically in an aquarium from day to night, so test periodically. I didn't mention it previously, but if you haven't already done so, you should look at pH.
