wiring IceCap ballast to PFO converter plug

I'm confused; how do I connect all the green wires? I think I figured out the rest, but I'm not sure about the ground wires.

Connect all three green wires together, or make sure each wire is grounded.

Do I have to screw them to the ballast housing too?

You can attach the green wire from the socket to the ballast. There should be a ground connection on the front of the ballast next to the wires.

What about the green wire from the ballast, the one that has a U shaped piece of metal at the end? I put all the greens together and clamped the U under a screw that holds the ballast to a piece of plywood. The whole thing (ballast, and the wood it's mounted on) sits inside a metal box with the ballast in contact with the box to draw extra heat out. Not sure if the whole thing's good or not.
